For anyone requesting a copy of treatment notes at the time of appointment or for any past appointments, please be aware that we follow HIPAA Compliance for record provision as well as the State of Oregon guidelines for document fees.
If you would like to receive a copy of your records, here are the steps you can take:
All requests must be made in writing. Email is acceptable. Please enquire if you would like a request slip during your appointment; or you may mail your request to the home office: 880 Nantucket Avenue, Eugene, OR 97404.
For copies of notes while you are at an appointment, the fee for an immediate print out is $5.00 and it is due upon receipt (this is for chart notes only – no other records will be available at the time of appointment).
For copies of your entire chart or any part based on a date range or topic, you need to contact my assistant by email at firstname.lastname@example.org or send a request through the US postal service to: 880 Nantucket Avenue, Eugene, OR 97404.
Please note: effective October 4, 2013, all treatment notes are electronically written. Treatment notes prior to October 4, 2013 are hand written and available. Transcription of the hand written notes is a separate fee. Please enquire with my assistant if you choose to have older treatment notes transcribed.
Copy & Mailing Fees, for an entire or partial chart, are based on number of pages copied as follows (ORS 192.521):
First ten pages – $30.00
Pages 11-50, additional $0.50 per page
Pages 50 and beyond, an additional $0.25 per page
(these fees are based on 2010 Oregon Statute and include the cost of postage for regular mail – if you require special mailing services, those additional fees will be added to the cost)
